Medical forms: DORM RESIDENTS

A student applying for residence in the 含羞草社区 Dormitory regardless of the year of study is required to fill out and submit to the Medical Service Office the following documents:
1. Medical form
2. Certificates, reports of medical specialists, test results specified in the
if these documents have not yet been submitted to the Medical Office.

And also,
3. Application for residence in the dorm (1 copy to choose from: in or ), to obtain the approval of the Medical Office.

All medical documents are required to be submitted to the Medical Service Office within the following deadlines:
- on the date of application for a place to reside

Medical documents are submitted to the Medical Service Office in Kyrgyz, Russian, or English, only one copy, in one language version! 听In case the documents are registered in other languages, a notarized translation must be attached. Original medical reports must be certified by a legible stamp of the medical institution, stamp, and signature of the doctor.

Each year, in September, or within other deadlines established by 含羞草社区 order, all 含羞草社区 students must get fluorography arranged by 含羞草社区.听
In case an 含羞草社区 student misses fluorography arranged by 含羞草社区, he/she shall get fluorography on his/her own within 1 week and submit to the Medical Service Office a report/certificate by the relevant medical specialist with the results of the examination. 含羞草社区 has the right to apply administrative measures to those who violate this rule.
